ENROLMENT REQUIREMENTS 2. Elementary students who are currently enrolled in a WRDSB
school, we only need a fully completed registration form. 3. Students who are
currently enrolled in another Board, including the Waterloo Catholic School
Board, must provide one of the following: a. Canadian
Birth Certificate or Baptismal Certificate b. Canadian
Passport (NOT a Canada
visa within a passport) c. Canadian
Citizenship Card
d. Confirmation of Permanent Residence Form - form 5292 or IMM1000 (must be verified with a passport) e. Permanent Resident Card f. Landed Immigrant papers Be sure to take a copy of BOTH sides of the provided document and attach it to the reg. form. 4. Credit class student need to provide OEN number. If a student is home schooled or comes from a private, unauthorized school, we will be required to obtain an OEN for him/her. If this is the case, please advise KWCS immediately so an OEN can be obtained. 5. Credit students
must provide a copy of a current transcript or CCS (Credit Counseling Summary)
which is available from their home school guidance office. A copy of this must be attached to the
registration form 6. Adults/Credit students wishing to attend an elementary class must pay a fee of $190. 填表后请于9月17日或24日早9:00-11:30到学校办公室交费. 支票Payable to "K-W Chinese School". Please make cheque payable to "K-W Chinese School" from 9am to 11:30am on Sept 17 or Sept 24, 2011 at main office of WCI.
